You can earn rewards by doing things that help you stay healthy.

To begin earning rewards, all you have to do is complete one of the health screenings, visits, or activities listed below.

Criteria for Enrollees to Participate

EPSDT/Well-child visit

Annual wellness visit
(ages 9 – 20)

 $50 All enrollees ages 9 – 20 are eligible to receive a $50 reward when they complete their annual wellness visit once between October 1, 2024 and September 30, 2025.
Diabetic blood and urine screening Blood (HbA1c) and urine (kidney health evaluation) tests for diabetics $25 All enrollees previously diagnosed with diabetes are eligible to receive a $25 reward when they complete an annual blood (HbA1C) and urine (kidney health evaluation) screening.
Diabetic retinal eye exam Retinal eye exam for diabetics $25 All enrollees previously diagnosed with diabetes are eligible to receive a $25 reward when they complete an annual retinal eye exam for diabetics.

Maternity prenatal visit

Maternity – prenatal visit

$25

All pregnant enrollees are eligible to receive a $25 reward when they complete a prenatal visit in their first trimester (before they are 13 weeks pregnant).

Maternity postpartum visit
Maternity – postpartum visit

$25

All enrollees who have delivered a baby are eligible to receive a $25 reward when they complete a postpartum visit between one to 12 weeks (seven to 84 days) after delivery.

*Limit for fiscal year (October 1, 2024 through September 30, 2025): The annual fiscal year limit is a total of $75 for any and all rewards/incentives, cumulatively. An individual can earn only one reward/incentive in each category per year.

Your CARE Card

Activation:

To activate your card, please call 1-888-682-2400 and follow the instructions. You can also activate your card by visiting www.myotccard.com. You may need your AmeriHealth Caritas District of Columbia (DC) ID card with you to complete activation.

Visit a participating retailer:

You can use your CARE Card at Walgreens, CVS Pharmacy, Rite Aid, and Walmart. With your CARE Card, you can buy products related to baby care, women’s care, diabetic supplies, pain relief, and more. Remember to take your card with you — you will need the card to pay for your eligible items.

Pay cashier:

Gather your eligible items and proceed to the store check-out lanes. When you check out, swipe the card for payment. With your CARE Card, you can pay for eligible items up to your available card balance.

Any remaining balance will carry over until your next purchase, and new rewards will be loaded as you earn them. You can only use your rewards while you are actively enrolled with AmeriHealth Caritas DC. Rewards expire if you lose AmeriHealth Caritas DC eligibility or are disenrolled from the plan.

*You cannot use your rewards to purchase alcohol, tobacco, or firearms. Rewards expire if you lose eligibility or are disenrolled from AmeriHealth Caritas DC for more than 60 days. Your rewards may not be converted to cash.

Eligible CARE Card Rewards Program rewards are subject to change. AmeriHealth Caritas DC will notify you before the change happens.

Once your health care provider notifies us that you have completed a healthy activity we will add rewards to your card. Enrollees may not be eligible to earn all of the rewards listed. For questions, call Enrollee Services at 1-800-408-7511 (TTY 1-800-570-1190), 24 hours a day, seven days a week.
